Berberine is an isoquinoline alkaloid found in several plants including Berberis vulgaris (barberry), Berberis aristata (Indian barberry), Coptis chinensis (goldthread), and Oregon grape. It has been used in traditional Chinese and Ayurvedic medicine for more than 2,500 years, and in the last two decades it has become one of the most-studied natural compounds in the metabolic health literature.

A widely cited 2008 clinical trial by Yin, Xing, and Ye published in Metabolism compared 500 mg of berberine taken three times daily to metformin in adults with type 2 diabetes. Over 3 months, both groups saw comparable reductions in fasting plasma glucose and hemoglobin A1c — a finding that helped ignite serious pharmacological interest in the molecule.

A 2015 meta-analysis in the Journal of Ethnopharmacology by Lan et al. pooled 27 randomized controlled trials and concluded that berberine, alone or with lifestyle intervention, produced meaningful improvements in fasting blood glucose, HbA1c, total cholesterol, and LDL-cholesterol compared with lifestyle intervention alone or placebo.

Mechanistically, berberine appears to activate AMP-activated protein kinase (AMPK), often called the body's "metabolic master switch." AMPK activation influences glucose uptake, fatty acid oxidation, and mitochondrial biogenesis — which helps explain the broad range of outcomes seen across cardiometabolic studies.

More recent work has explored berberine's effects on the gut microbiome. A 2020 review in Frontiers in Pharmacologysummarized evidence that berberine's poor oral bioavailability may actually be a feature, not a bug — it stays in the gut long enough to interact with microbial populations linked to metabolic and inflammatory health.
